ABSTRACT

OBJECTIVETo compare the efficacy difference between fire acupuncture with centro-square needles (FACSN) and filiform needling (FN) for knee osteoarthritis (KOA).

METHODSSeventy-two patients were randomly assigned into an FACSN group and an FN group, 36 cases in each one.points, Xuehai (SP 10), Liangqiu (ST 34), Neixiyan (EX-LE 4), Dubi (ST 35), Zusanli (ST 36), Yanglingquan (GB 34) and Yinlingquan (SP 9) were selected in the two groups. The FACSN group was treated with FACSN, and three acupoints were selected for each treatment; the FN group was treated with FN, and all the acupoints were selected for each treatment. The cupping treatment was given after acupuncture in the two groups. The treatment was given once every other day, without treatment on Sundays. The treatment was given three times a week, 6 times as one course; totally 2 courses were provided. The visual analogue scale (VAS) and Western Ontario and McMaster Universities Arthritis Index (WOMAC) were observed in the two groups before treatment, two weeks, four weeks into treatment and at one-month follow-up visit. In addition, the comprehensive efficacy was compared between the two groups.

RESULTSCompared before treatment, the score of VAS and the total score of WOMAC were improved in the two groups at each time point after treatment (all<0.01); the scores of VAS at each time point after treatment in FACSN group were lower than those in the FN group (all<0.05); four weeks into treatment and at one-month follow-up visit, the total score of WOMAC in the FACSN group was lower than that in the FN group (both<0.05). Two weeks into treatment, the total effective rate was 88.9% (32/36) in the FACSN group, which was higher than 61.1% (22/36) in the FN group (<0.01); four weeks into treatment and at one-month follow-up visit, the cured and remarkable effective rates were 66.7% (24/36) and 83.3% (30/36) in the FACSN group, which were higher than 41.7% (15/36) and 44.4% (16/36) in the FN group (<0.05,<0.01), respectively.

CONCLUSIONSFire acupuncture with centro-square needles has relatively high cured and remarkable effective rate for KOA, with rapid onset; as for pain relief, the efficacy is superior to filiform needling.

ABSTRACT

syndrome is classified into five categories. Based on the syndrome differentiation of meridians and collaterals andtheories, Professorproposed the fire needling therapy at the lower-sea points for five types ofsyndromes. Professorstresses on the flexible application of fire needling therapy forsyndrome at the lower-sea points and innovated the unique five techniques of fire needling therapy, named lifting and threating needle of small amplitude at the lower-sea points, the stimulation along the relevant meri-dians before fire needling, detecting the sensitive sites by gentle pressing the relevant lower-sea points, combining the movement of affected upper limb or joint during fire needling, or combining with contralateral needling technique, and controlling the depth of fire needling based on the duration of sickness and depth of affected site. He pointed out that Shangjuxu (ST 37) is for skinsyndrome, Zusanli (ST 36) for musclesyndrome, Yanglingquan (GB 34) for tendonsyndrome, Weizhong (BL 40) for bonesyndrome and Xiajuxu (ST 39) for vesselsyndrome.

ABSTRACT

Objective To evaluate if 18F-FLT PET imaging could be used as a new clinical method to predict tumor radiosensitivity. Methods MDA-MB-231 and LN229 cells were irradiated with doses of 0,8 and 16 Gy of 6 MV photon energy, then soft agar assay and cellular uptake of 18F-FLT were performed on the 2 cell lines. The t test and one-way analysis of variance were used for the two groups and data before and after irradiation. The MDA-MB-231 and LN229 tumor xenografts were prepared by injecting the tumor cells into the right limbs of female BALB/c nu/nu mice.Once tumors reached a diameter of 10 mm,the two types of mice were divided randomly into 3 groups (20 mice per group) according to the irradiation doses (0,8 and 16 Gy).After irradiation,18F-FLT PET imaging and immunohistochemical staining were conducted. Then correlations between 18F-FLT SUVtumor/SUVmuscle ratio (T/M ratio) and TK1 labeling index percentages (LITK1) were tested using linear correlation analysis. Results The survival fraction of MDA-MB-231 and LN229 cells after irradiated with 8 Gy were (59.73 ± 4.3) % and (93.41 ± 3.75) %,respectively (t =-13.20,P < 0.001).When the dose increased to 16 Gy,the survival fraction decreased to (43.57 ±4.06) % and (81.77 ± 4.42) %,respectively(t =-14.24,P < 0.001).In MDA-MB-231 cells, the cellular uptake of 18F-FLT after irradiation with 8 Gy declined rapidly to (18.32 ± 1.38) kBq/105 cells ((128.22 ± 8.24) kBq/105 cells with the dose of 0 Gy,F =266.41,P < 0.01),and maintained this low level till 72 h.For the LN229 cells, the cellular uptake decreased to (9.87 ± 1.30) kBq/105 cells after 8 Gy irradiation ((134.88 ± 6.59) kBq/105 cells with the dose of 0 Gy,F =346.06,P < 0.01),then increased gradually to (127.17 ± 9.08) kBq/105 cells at 72 h (F =346.06,P > 0.05).The dynamic changes of 18F-FLT cellular uptake in the two cells had the same pattern after being treated with 16 Gy irradiation. In the 18F-FLT PET image of MDA-MB-231 tumor mice after 8 Gy radiotherapy, the T/M ratio decreased to 0.78 ± 0.39 at the first day,but it was 2.84 ± 0.29 before radiotherapy (F =39.78,P <0.01).Then the ratio increased slowly, and it was still lower than the baseline at 7 d after radiation (F =39.78,P <0.01).The same pattern could be seen in the group of 16 Gy irradiation. In LN229 tumor mice treatment with 8 Gy irradiation, the T/M ratio increased to 2.41 ±0.47 at the first day,and it was 1.58 ±0.29 before radiotherapy (F =34.01,P < 0.05).The ratio decreased steadily to 0.66 ± 0.32 (F =34.01,P<0.05) at 7 d after radiotherapy. However,in the treatment group with 16 Gy,the T/M ratio decreased gradually and reached 0.44 ± 0.22 at 7 d (F =41.85,P < 0.01).A correlation was found between 18F-FLT T/M ratio and LITK1 (8 Gy:r=0.67,0.73; 16 Gy:r=0.73,0.69; all P<0.01) in both tumor models. Conclusion 18F-FLT PET imaging may be used as a new assay to predict tumor radiosensitivity, but further investigation is needed before clinical application.

ABSTRACT

Objective To explore the common causes of epilepsy and the etiologic characteristics in different age groups of patients with epilepsy. Methods A retrospective survey was made in 5572 epilepsy patients in Epileptic Center of Guangdong 999 Brain Hospital from January 2003 to December 2009.According to the diagnostic criteria published in 2005 from ILAE,all the diagnoses of 5572 cases were made by epileptic specialists.Based on history, cranial MRI or CT and pathologic data, causes of epilepsy were classified into idiopathic, symptomatic and cryptogenic epilepsy. The cases of symptomatic epilepsy were further arranged into different categories in different age grades, such as head trauma, perinatal injuries, infection in central nervous system, cerebral vascular disease, brain tumor, disorders of cortical development, neurocutaneous syndrome and others. The cases with febrile seizures and family history were collected, and positive ratio of febrile seizures and family history were contrasted in different categories of cases by Kruskal-Wallis test ( nonparametric test ).Results In 5572 cases,66 were idiopathic,2834 symptomatic,2672 cryptogenic, and the ratio of these causes was 1%,51%,48% respectively.Among 2834 cases of symptomatic epilepsy,822 were head trauma,497 were perinatal injuries,360 were infection in central nervous system,249 were brain tumor,150 were cerebral vascular disease,135 were disorders of cortical development,62 were neurocutaneous syndrome and 559 were others. In brief, head trauma, perinatal injuries, infection in central nervous system, brain tumor and cerebral vascular disease were top 5 causes of symptomatic epilepsy. Hippocampal sclerosis was found in 744 cases in those of eryptogenic epilepsy. The importance of febrile seizures( idiopathic:15.2% ( 10/66 ),symptomatic:6.5% ( 185/2834 ),cryptogenic:9.4% ( 250/2672 ) ; x2 =181.393,P =0.000 ) and family history ( idiopathic:83.3% ( 55/66 ),symptomatic:1.1% (31/2834),cryptogenic:0.4% (12/2672) ; x2 =68.354,P =0.000) was statistically different in different causes of epilepsy.Febrile seizures was the most frequent in cases with hippocampal sclerosis than those with other causes, and family history was the most frequent in neurocutaneous syndrome in symptomatic cases.Perinatal injurics was thc first causc in cases of infancy and childhood, head trauma was the top one in those of juvenile and adulthood, and cerebral vascular disease was the main cause in senile cases. Conclusions In the whole epileptic cases of 5572, 1% was idiopathic,51% was symptomatic, and 48% cryptogenic. The main causes of them were head trauma, perinatal injuries, infection in central nervous system, brain tumor, and cerebral vascular disease.

ABSTRACT

3′-deoxy-3′-18F-fluorothymidine has been described recently as a new positron emission tomography (PET) tracer for imaging tumor cell proliferation. In recent years, more and more clinical research demonstrate that 18F-FLT can image tumor cell proliferation during treatment, and can reflect accelerated reproliferation earlier. it can define tumor subvolumes with high proliferative activity preceding volumetric tumor response, and can predict relapse and prognosis according to the changing status during the treatment.
